 Original Medical Receipt(s) and Invoice

 Original Funeral Expenses Receipt(s)  

 Post Mortem Report

 Police Report from the Parents about the Accident

 Police Report from Other Parties involved in the Accident (if any)  

 UTAR Offer Letter of Admission to the Student  

 Confirmation Letter of Student's Status from the Faculty

 Student Bill & Payment Receipt

 E-Payment Form (Download HERE)

 One of the following items:

   Parent's Bank Account Statement (First Page), OR 

   Parent's Bank Passbook showing the Account Name and Number, OR

   Parent's Bank Account Details obtained from Bank's website certified by the respective bank, OR

   Letter from the Bank confirming the Parent's Bank Account details
