 Photocopy of Student's MyKad / Passport

 Photocopy of Student's ID Card

 Original Medical Receipt(s)

 Photograph(s) and/or X-ray report to indicate severed body part(s)

 Police Report about the Accident 

 Confirmation Letter of Student's Status from the Faculty

 Student Bill Payment Receipt

 E-Payment Form (Download HERE)

 One of the following items:

   Student's Bank Account Statement (First Page), OR 

   Student's Bank Passbook showing the Account Name and Number, OR

   Student's Bank Account Details obtained from Bank's website certified by the respective bank, OR

   Letter from the Bank confirming the Student's Bank Account details
